Youth Partner Care Manager

The Youth Partner (YP) is responsible for supporting Chosen’s mission to help youth heal from trauma and those impacted by the child will-being system by using its unique, trauma-informed model. The YP coordinates the overall plan of care for the client, from inquiry to completion, and works directly with youth while providing coaching, case management, and therapeutic resources. The position involves a multi-disciplinary approach and serves high acuity youth in the child well-being system.
